University of Southampton Module Evaluation Form
This is the full Module Evaluation Form adopted by the University of Southampton. The latest editable file can be downloaded from the Learning and Teaching Enhancement Unit (LATEU) of the University. Included in this resource is the online version of the form for use in Blackboard, WebCT and other virtual learning environments. If you are using Blackboard, you are advised to use Internet Explorer version 6 or higher. Save the Blackboard zip archive to a local drive. Do not rename the file name. Go to the destination course area in Blackboard, open the "Control Panel" and then start the "Survey Manager" (in the "Assessment" group). Use the "Import" command to upload the zip archive. Once this is completed, rename the evaluation form which can then be added to any content area within the course using the dropdown "Add Survey" command.
Advice for reuse
This is a full module evaluation form. At the School of Geography only Questions 17 - 28 are presented to students and the form is embedded as a Blackboard survey in each of the teaching modules. The summary of the results of previous years can be found in one of the links provided. For those who would like to use the online version of the form, make sure you download the zip that is compatible with your institution's learning management system (e.g. Blackboard, WebCT or text). The Respondus version contains plain text that is correctly formatted for use with the Respondus quiz editing tool. Respondus allows users to alter and upload the questions to a wide range of virtual learning environments including Blackboard, WebCT and any systems that are compatible with the IMS-QTI standard.
